Schedule:
- Flexible shifts available Monday through Sunday
- Ability to create your own schedule
- Minimum weekly or bi-weekly hourly commitment required
Key Responsibilities:
- Perform comprehensive home health risk assessments for Medicare Advantage members
- Conduct thorough reviews of prior and current medical history, conditions, and medications
- Educate patients about chronic conditions, treatment compliance, and medication use
- Identify gaps in care and recommend appropriate resolutions
- Maintain timely and accurate documentation following assessment protocols
- Note: No treatment plan creation or medication prescribing involved
Qualifications:
- Current Nurse Practitioner license in Arkansas
- Board Certification through AANP or ANCC
- MSN degree from an accredited nursing program
- Valid CPR certification (BLS)
- At least 6 months of recent Nurse Practitioner experience preferred
