Advance your career as a Nurse Practitioner specializing in home health risk assessments for Medicare Advantage plan members in Washington County, Arkansas. This per diem opportunity focuses on providing high-quality care through comprehensive annual and post-discharge assessments conducted in patients homes. Service areas include Fayetteville, Springdale, and Farmington, offering diverse experiences across the region.

- Flexible shifts available Monday through Sunday

- Commitment to a minimum number of hours per week or bi-weekly
Core Responsibilities:
- Perform thorough home health risk assessments for Medicare Advantage members, with each visit lasting about 45 minutes
- Review and evaluate patients medical history, current health status, and medication regimens
- Deliver patient education regarding health conditions and prescribed medications
- Identify care gaps and recommend appropriate actions to address them
- Prescribing medications or developing treatment plans is not part of this role
Qualifications:
- Active Nurse Practitioner license in Arkansas
- Current BLS/CPR certification
- Board Certification via AANP or ANCC
- Master of Science in Nursing (MSN) from an accredited institution
- Minimum of 6 months NP experience preferred
