A fulfilling per diem opportunity awaits Nurse Practitioners seeking dynamic, patient-centered work in home health. This position involves performing comprehensive risk assessments for Medicare Advantage plan members living within Washington County, ARincluding Fayetteville, Springdale, and Farmington. Assignments are conducted in patients homes, with each visit lasting about 45 minutes. Shifts are scheduled based on your availability, making this role ideal for practitioners interested in flexibility and independence.
Schedule:
- Flexible shifts available Monday through Sunday
- Set your own schedule
- Minimum weekly or bi-weekly hour commitment
Core Responsibilities:
- Perform annual home health risk assessments with a focus on Medicare Advantage members
- Review and evaluate current and previous medical history, health conditions, and medication regimens
- Offer education to members on their medical conditions and prescribed medications
- Identify gaps in member care and provide recommendations for improved health outcomes
- No requirement to create treatment plans or prescribe medications
Desired Qualifications:
- Active Nurse Practitioner license in Arkansas
- Board Certification via AANP or ANCC
- Current BLS/CPR certification
- Master of Science in Nursing (MSN) from an accredited program
- At least 6 months of recent Nurse Practitioner experience preferred
