Enhance patient care in a dynamic, flexible home health setting as a Nurse Practitioner specializing in risk assessments. This per diem opportunity centers around conducting home health risk evaluations for Medicare Advantage plan members across Cumberland County, including communities such as Spring Lake, Hope Mills, and Eastover. With each assessment typically lasting about 45 minutes, you will have the opportunity to make a significant difference in patients overall well-being through independent visits directly in their homes.
Schedule:
- Flexible shifts available Monday through Sunday
- Self-scheduling to fit your availability
- Minimum weekly or bi-weekly hourly commitment required
Key Responsibilities:
- Complete annual home health risk assessments in patient residences
- Evaluate comprehensive medical, surgical, and medication histories
- Educate members about medical conditions and prescribed medications
- Identify and communicate care gaps, recommending steps for resolution
- No need to create detailed treatment plans or prescribe medications
Qualifications:
- Active Nurse Practitioner license in North Carolina
- Board Certification via AANP or ANCC required
- Current CPR (BLS) certification
- Master of Science in Nursing (MSN) from an accredited institution
- At least 6 months of current experience as a Nurse Practitioner preferred
