Job Description

Seeking an experienced Nurse Practitioner to perform home health risk assessments throughout Muskegon County, MI. In this per diem position, you will provide crucial evaluations for Medicare Advantage plan members, aiding in comprehensive health management without the need for ongoing treatment planning or prescribing. The work environment is flexible and autonomousideal for those seeking to design their own schedule.

Schedule:

  • Flexible shifts available Monday through Sunday
  • Self-scheduling to fit your availability
  • Options for minimum weekly or bi-weekly hourly commitments

Key Responsibilities:

  • Conduct annual home health risk assessments with an average visit time of 45 minutes
  • Travel to members homes within Muskegon County, including Muskegon, Norton Shores, and Muskegon Heights
  • Review current and prior medical history, conditions, and medications
  • Educate members regarding their health and medications
  • Identify care gaps and recommend next steps or additional resources as appropriate
  • No ongoing treatment plans or medication prescribing required

Qualifications:

  • Active Nurse Practitioner (NP) license in Michigan
  • Master of Science in Nursing (MSN) from an accredited institution
  • Current CPR certification (BLS)
  • Board certification from AANP or ANCC
  • At least six months of recent nurse practitioner experience preferred

Compensation for this position is $80-$90 per completed evaluation, with added reimbursement for mileage and administrative time.
