Deliver comprehensive patient care as you conduct home health risk assessments for Medicare Advantage members across Williamson County. This per diem opportunity empowers you to make a meaningful difference in the lives of patients residing in various communities, with flexibility to tailor your work around your schedule.
Schedule:
- Flexible shifts available between Monday and Sunday
- Self-schedulingdesign your own work calendar
- Minimum weekly or bi-weekly hourly commitment
Key Responsibilities:
- Complete annual home health risk assessments for Medicare Advantage health plan members in their homes
- Review and evaluate medical histories, conditions, and medication usage
- Deliver patient education regarding chronic conditions, medications, and preventive care
- Identify gaps in care and recommend appropriate actions
- No requirement for developing treatment plans or prescribing medications
Qualifications:
- Active Nurse Practitioner license in Tennessee
- Board Certification by AANP or ANCC
- Current CPR certification (BLS)
- MSN degree from an accredited nursing program
- At least 6 months of recent experience as a Nurse Practitioner preferred
