Job Description

Deliver comprehensive patient care as you conduct home health risk assessments for Medicare Advantage members across Williamson County. This per diem opportunity empowers you to make a meaningful difference in the lives of patients residing in various communities, with flexibility to tailor your work around your schedule.

Schedule:

  • Flexible shifts available between Monday and Sunday
  • Self-schedulingdesign your own work calendar
  • Minimum weekly or bi-weekly hourly commitment

Key Responsibilities:

  • Complete annual home health risk assessments for Medicare Advantage health plan members in their homes
  • Review and evaluate medical histories, conditions, and medication usage
  • Deliver patient education regarding chronic conditions, medications, and preventive care
  • Identify gaps in care and recommend appropriate actions
  • No requirement for developing treatment plans or prescribing medications

Qualifications:

  • Active Nurse Practitioner license in Tennessee
  • Board Certification by AANP or ANCC
  • Current CPR certification (BLS)
  • MSN degree from an accredited nursing program
  • At least 6 months of recent experience as a Nurse Practitioner preferred
