Advance your career by conducting meaningful home health risk assessments for Medicare Advantage plan members in the vibrant communities of Maury County, TN, including Columbia, Spring Hill, and Mount Pleasant. As a per diem Nurse Practitioner, you will complete comprehensive in-home visits, each averaging about 45 minutes, for patients recently discharged or needing their annual assessment.

Key Responsibilities:
- Perform thorough annual risk assessments as part of a Medicare Advantage health plan
- Review patients medical history, current conditions, and medication lists to ensure holistic evaluations
- Educate members about their health conditions, medications, and preventive care
- Identify gaps in care and provide actionable recommendations for improved health outcomes
- No treatment planning or prescribing requiredfocus is on assessments and education
Qualifications:
- Active Nurse Practitioner license in Tennessee
- Current Basic Life Support (CPR) certification
- Board certification through AANP or ANCC
- Masters degree (MSN) from an accredited nursing program
- At least six months of recent Nurse Practitioner experience preferred
This opportunity offers compensation of $80$90 per completed evaluation, along with a mileage stipend for travel and additional pay for administrative time.
