As a Nurse Practitioner specializing in home health, you will play a vital role in assessing and supporting Medicare Advantage members within Maury County, TN. Your expertise will directly contribute to quality care by performing risk assessments in patients homes across Columbia, Spring Hill, and Mount Pleasant. This per diem position offers exceptional flexibility, allowing you to set your own schedule and choose shifts that work best for you.
Schedule:
- Flexible shifts available Monday through Sunday
- Self-schedulingcreate your own work hours
- Minimum weekly or bi-weekly hourly commitment, as preferred
Core Responsibilities:
- Conduct home health risk assessments, generally taking 45 minutes per visit
- Review each individual's past and current medical history, medical conditions, and medication use
- Offer education on medical conditions and medication management to promote patient understanding
- Identify potential care gaps and recommend solutions to improve patient outcomes
- No responsibility for prescribing medications or devising treatment plans
Qualifications:
- Active Nurse Practitioner License in Tennessee
- Current CPR certification (BLS required)
- Board Certified through either AANP or ANCC
- Preferred: 6 months of current NP experience
- MSN degree from an accredited nursing school
Youll be compensated $80-$90 per evaluation, alongside a mileage stipend and paid administrative time, recognizing the value of your experience and dedication.
