Schedule:
- Flexible shifts available Monday through Sunday
- Self-scheduling: create your own work schedule
- Minimal weekly or bi-weekly hourly commitment required
Key Responsibilities:
- Perform annual home health risk assessments for Medicare Advantage plan members in various locations within Spotsylvania County
- Evaluate patients medical history, current conditions, and medication use during each 45-minute assessment
- Provide education regarding health status, conditions, and medications
- Identify any gaps in care and make recommendations to support the care plan
- Visits include a range of assessment types, such as annual checkups or post-discharge reviews
- Travel within the county, including areas like Spotsylvania Courthouse and Lake Wilderness
- No requirement to develop treatment plans or prescribe medications
Qualifications:
- Valid and unrestricted Nurse Practitioner license in Virginia
- Current BLS (CPR) certification
- Board Certification through AANP or ANCC
- MSN from an accredited nursing program
- At least 6 months of recent experience as a Nurse Practitioner preferred
