Make a meaningful impact in patient care by performing essential home health risk assessments for Medicare Advantage members throughout Le Flore County. This per diem opportunity is ideal for Nurse Practitioners passionate about providing clinical insights and comprehensive health education without the responsibility of ongoing treatment or prescribing.

Schedule:

Primary Responsibilities:

• Conduct detailed annual home health risk assessments in members homes
• Review and evaluate prior and current medical histories, chronic conditions, and medication regimens
• Deliver clear education and counseling regarding conditions and medication management
• Identify care gaps and supply actionable recommendations for improved health outcomes
• Visits vary and may include annual assessments or post-discharge evaluations
• No prescribing of medication or creation of ongoing treatment plans required

Qualifications:

• Active Nurse Practitioner License in Oklahoma
• Current CPR certification (BLS)
• Board Certification through either AANP or ANCC
• Minimum of six months recent NP experience preferred
• MSN degree conferred by an accredited nursing program
