Job Description
Deliver proactive care as an experienced Nurse Practitioner specializing in home health risk assessments for Medicare Advantage plan members in White County, TN. In this per diem role, you will make a meaningful impact by visiting patients in their homesprimarily Sparta, Doyle, and surroundsensuring comprehensive and compassionate health evaluations.
Schedule:
- Flexible shifts available Monday to Sunday
- Freedom to design your own work schedule
- Minimum weekly or bi-weekly hourly commitment required
Responsibilities:
- Complete annual home health risk assessments for Medicare Advantage members
- Assess patients' prior and current medical history, current conditions, and medication usage
- Offer patient education regarding diagnoses and medications
- Identify and address gaps in care with detailed recommendations for improvement
- Perform post-discharge home visits as needed
- Note: This role does not involve creating treatment plans or prescribing medications
Qualifications:
- Active Nurse Practitioner license in the State of Tennessee
- Current Basic Life Support (CPR/BLS) certification
- Board-certified through AANP or ANCC
- Master of Science in Nursing (MSN) from an accredited institution
- At least 6 months of recent experience as a Nurse Practitioner preferred
