Elevate your nursing career with a per diem opportunity focused on home health assessments for Medicare Advantage members. As a Nurse Practitioner specializing in home health, you will conduct focused, patient-centered risk assessments within homes throughout White County, including Sparta and Doyle. Each assessment visit averages 45 minutes and may involve an annual review or a post-discharge evaluation, dedicated to improving patient outcomes and facilitating seamless healthcare transitions.
Schedule:
- Flexible shifts available Monday through Sunday
- Design your own schedule for optimal work-life balance
- Minimum weekly or bi-weekly hourly commitment
Key Responsibilities:
- Perform annual home health risk evaluations for Medicare Advantage plan members
- Review medical and medication histories, as well as prior and current conditions
- Educate patients about medical diagnoses and medication regimens
- Identify care gaps and make actionable recommendations to bridge them
- No requirements for developing treatment plans or prescribing medications
Qualifications:
- Active Nurse Practitioner license in Tennessee
- Current CPR certification (BLS)
- Board Certification through AANP or ANCC
- MSN from an accredited nursing program
- At least 6 months of current experience as an NP preferred
This position offers a competitive compensation structure of $80$90 per completed evaluation, supplemented by a mileage stipend and pay for administrative time.
