Job Description
Advance your career as a Nurse Practitioner by providing meaningful care through home health risk assessments for Medicare Advantage plan members in White County, TN, including Sparta and Doyle. This per diem role offers the flexibility to create your own schedule with shifts available any day of the week.
Schedule
- Choose shifts Monday through Sunday
- Set your own work schedule
- Commit to weekly or bi-weekly hours as fits your availability
Key Responsibilities
- Complete annual and post-discharge home health risk assessments (average 45 minutes each)
- Review medical history, current health status, and medication use for patients in their homes
- Educate patients on their health conditions and prescribed treatments
- Identify care gaps and recommend actionable steps to close them
- No requirement to develop treatment plans or prescribe medications
Qualifications
- Active Nurse Practitioner license in TN
- Board Certification through AANP or ANCC
- Current CPR certification (BLS required)
- MSN from an accredited nursing program
- At least 6 months experience as an NP preferred
