Job Description

Make a meaningful impact on patient care by performing home health risk assessments for Medicare Advantage members in Putnam County. This per diem opportunity allows nurse practitioners to use their clinical expertise while enjoying an exceptionally flexible schedule and the autonomy to manage their caseload.

Schedule:

  • Flexible shifts available Monday through Sunday
  • Build your own schedule
  • Hourly commitments are required on a weekly or bi-weekly basis

Key Responsibilities:

  • Conduct comprehensive annual risk assessments in patients homes
  • Evaluate patient medical histories, ongoing conditions, and review medication regimens
  • Educate patients about their diagnoses and prescribed medications
  • Identify gaps in care and provide actionable recommendations to promote improved outcomes
  • Assessments onlyno responsibility for creating treatment plans or prescribing medications

Qualifications:

  • Active Nurse Practitioner license in Tennessee
  • Current CPR certification (BLS required)
  • Board certified through AANP or ANCC
  • MSN degree from an accredited nursing program
  • Minimum of 6 months current nurse practitioner experience preferred

Enjoy competitive earnings with $80-$90 per completed evaluation, plus a mileage stipend and pay for administrative time. Assignments are within Putnam County, including Cookeville, Algood, and Monterey. Each home visit typically lasts about 45 minutes, focusing on either annual health assessments or post-discharge follow-ups.
