Step into a meaningful role delivering home health assessments to Medicare Advantage plan members in Putnam County, TN. This per diem opportunity is ideal for Nurse Practitioners seeking a flexible schedule while providing important care within the community.
Schedule:
- Flexible shifts available Monday through Sunday
- Set your own schedule to fit your lifestyle
- Minimum weekly or bi-weekly hourly commitment required
Responsibilities:
- Perform in-home annual health risk assessments for Medicare Advantage members
- Review prior and current medical history, conditions, and medication usage
- Deliver patient education regarding medical concerns and medications
- Observe and identify gaps in care, making recommendations to address these gaps
- No need to create treatment plans or prescribe medications
- Complete visits throughout Putnam County, including Cookeville, Algood, and Monterey
Qualifications:
- Active Nurse Practitioner license in Tennessee
- Board Certification by the AANP or ANCC
- Master of Science in Nursing from an accredited institution
- Current CPR (BLS) certification
- At least 6 months of recent NP experience preferred
- Skilled at independent decision-making in the field
