Advance your nursing career with a dynamic per diem role focused on providing home health risk assessments for Medicare Advantage members throughout Washoe County, NV. As a Nurse Practitioner, you will have the autonomy to create your own schedule while making a meaningful impact in the lives of patients in their own homes, including areas such as Reno, Sun Valley, and Spanish Springs.

Schedule:

• Flexible shifts available Monday through Sunday
• Design your own schedule
• Committed minimum hours on a weekly or bi-weekly basis

Key Responsibilities:

• Conduct comprehensive risk assessments in patient homesvisits average 45 minutes each
• Review and evaluate prior/current medical history, conditions, and medication use
• Provide clear education to patients on their medical conditions and prescribed therapies
• Identify care gaps and make recommendations to improve patient outcomes
• No requirement to create treatment plans or write prescriptions

Qualifications:

• Active Nurse Practitioner license in NV
• Current CPR certification (BLS)
• Board Certification through AANP or ANCC
• Minimum of 6 months current experience as an NP preferred
• MSN from an accredited school of nursing
