Job Description

This per diem opportunity is ideal for an experienced Nurse Practitioner passionate about delivering quality care in a home health setting. You will perform comprehensive risk assessments for Medicare Advantage plan members throughout Clark County, ARincluding Arkadelphia, Gurdon, and Amity. The average assessment lasts approximately 45 minutes and may encompass annual check-ups or post-discharge visits. Each day offers variety, with the flexibility to create your own schedule and make a meaningful impact in your community.

Schedule:

  • Flexible shifts available Monday through Sunday
  • Customize your schedule based on availability
  • Choose a weekly or bi-weekly minimum hourly commitment

Key Responsibilities:

  • Complete annual home health risk assessments for Medicare Advantage members
  • Review prior and current medical history, ongoing conditions, and medications
  • Educate patients on their health, chronic conditions, and prescriptions
  • Identify gaps in care and recommend actionable solutions
  • Note: No treatment planning or medication prescribing required

Qualifications:

  • Active Nurse Practitioner License in Arkansas
  • Board Certification through AANP or ANCC
  • Current BLS/CPR certification
  • MSN from an accredited nursing program
  • At least 6 months of recent Nurse Practitioner experience preferred
