Take on a dynamic per diem role as a Nurse Practitioner focused on home health risk assessments in the Spanaway and greater Thurston County areaincluding Lacey, Olympia, and Tumwater. In this position, youll enjoy flexibility while conducting thorough risk evaluations for Medicare Advantage plan members in their own homes. Each visit, averaging about 45 minutes, may range from comprehensive annual assessments to specific post-discharge reviews.

Schedule:

• Flexible shifts available seven days a week
• Create your own schedule to fit your availability
• Minimum weekly or bi-weekly hourly commitment

Key Responsibilities:

• Perform detailed annual risk assessments for Medicare Advantage members in their homes
• Review prior and current medical history, ongoing conditions, and documented medications
• Deliver patient education about health conditions and prescribed medications
• Identify and document care gaps, with recommendations for ongoing management
• No requirement for creating treatment plans or prescribing medications

Qualifications:

• Active Nurse Practitioner license in Washington State
• Current CPR certification (BLS)
• Board certification via AANP or ANCC
• At least six months of experience as a Nurse Practitioner is preferred
• Master of Science in Nursing (MSN) from an accredited institution
