Deliver quality patient care in a per diem role providing in-home health risk assessments for Medicare Advantage members across Cumberland County, ME. This opportunity offers the flexibility to create your own schedule, with shifts available throughout the week, including weekends. Assignments may vary day to day and may include locations such as Portland, South Portland, and Westbrook.

Schedule:

• Flexible scheduling: shifts available Monday through Sunday
• Set your own work hours
• Meet minimum weekly or bi-weekly hour commitments

Key Responsibilities:

• Conduct comprehensive home health risk assessments for Medicare Advantage plan members
• Review medical, surgical, and medication history with patients
• Provide patient education about medical conditions and prescribed therapies
• Identify gaps in patient care and offer appropriate recommendations
• Complete thorough documentation following each assessment
• Duties do not include developing treatment plans or prescribing medications

Qualifications:

• Unrestricted Nurse Practitioner license in ME
• MSN from an accredited school of nursing
• Active board certification (AANP or ANCC)
• Current BLS (CPR) certification
• Preferably at least 6 months of recent NP experience, ideally in home health, primary care, or geriatrics

Compensation ranges from $80 to $90 per completed evaluation and includes a mileage stipend as well as paid administrative time.
