Deliver high-impact care as a Nurse Practitioner providing in-home health risk assessments for Medicare Advantage plan members within Putnam County, including Hurricane, Winfield, and Eleanor. This flexible per diem opportunity enables you to make a meaningful difference while enjoying the freedom to craft your own work schedule.

Schedule:

• Flexible shifts available Monday through Sunday
• Choose your own hours and build your ideal schedule
• Meet a minimum weekly or bi-weekly hourly commitment

Responsibilities:

• Perform thorough annual or post-discharge home health risk assessments for Medicare Advantage members
• Review patients medical history, current conditions, and medication usage
• Educate members about their medical conditions and prescribed medications
• Identify and communicate gaps in care, offering actionable recommendations
• Focus exclusively on clinical assessment; no need to develop treatment plans or prescribe medications

Qualifications:

• Active Nurse Practitioner license in West Virginia
• Current BLS (CPR) certification
• Board certified by either AANP or ANCC
• At least 6 months of recent NP experience (preferred)
• Master of Science in Nursing (MSN) from an accredited program
