Job Description

Deliver compassionate, patient-focused care as a Nurse Practitioner specializing in Home Health. This per diem opportunity offers the perfect blend of autonomy and flexibility, empowering you to design your own schedule while impacting the lives of Medicare Advantage members in South Charleston and throughout Wood County, WV, including Parkersburg, Vienna, and Williamstown.

Schedule:

  • Flexible shifts available any day, Monday through Sunday
  • Self-scheduling: set your preferred days and hours
  • Choose a minimum weekly or bi-weekly hourly commitment to suit your availability

Key Responsibilities:

  • Perform comprehensive home health risk assessments for Medicare Advantage members
  • Complete detailed reviews of medical history, current medical conditions, and medication regimens
  • Educate patients regarding their health conditions and prescribed medications
  • Identify care gaps and recommend appropriate next steps or follow-up
  • Focus solely on evaluationno treatment planning or medication prescribing required
  • Average visit time is approximately 45 minutes per assessment

Qualifications:

  • Current, active Nurse Practitioner license in West Virginia
  • Board Certification through AANP or ANCC
  • MSN degree earned from an accredited nursing program
  • Current BLS CPR certification
  • At least 6 months of recent Nurse Practitioner experience preferred

This position provides a pay structure of $80$90 for each evaluation completed, in addition to a mileage stipend and compensation for administrative time.
