Advance your nursing career by providing vital care in a dynamic per diem position, performing home health risk assessments for Medicare Advantage members in South Charleston and the surrounding areas of Mercer County, WV.

Schedule:

• Flexible shifts available Monday through Sunday
• Design your own work schedule
• Commit to a minimum number of weekly or bi-weekly hours

Responsibilities:

• Conduct comprehensive annual home health risk assessments for Medicare Advantage plan participants in their homes
• Review prior and current medical histories, chronic and acute medical conditions, and medication regimens
• Deliver clear education to members about health conditions and proper medication use
• Identify care gaps and suggest actionable recommendations to enhance patient health outcomes
• Visits may include annual health evaluations or post-discharge assessments, with each visit taking roughly 45 minutes
• No prescription writing or treatment plan development required

Qualifications:

• Active Nurse Practitioner license (WV)
• Current BLS (CPR) certification
• National board certification as an NP (AANP or ANCC)
• MSN degree from an accredited nursing program
• At least 6 months of recent experience as a Nurse Practitioner preferred

In this role, youll earn between $80 and $90 per completed evaluation, along with a mileage stipend and compensation for administrative time.
