Seeking a dedicated Nurse Practitioner to perform home health risk assessments for Medicare Advantage plan members in Pacific County, Washington, including Raymond, South Bend, and Long Beach. This per diem opportunity is ideal for professionals looking to create a flexible work schedule while delivering meaningful care directly in patients homes.
Schedule:
- Flexible shifts available Monday through Sunday
- Design your own schedule to fit your lifestyle
- Choose your own hourly commitment on a weekly or bi-weekly basis
Key Responsibilities:
- Complete annual risk assessments during 45-minute home visits, including annual health or post-discharge assessments
- Evaluate patients prior and current medical histories, medical conditions, and medications
- Educate members about their health conditions and medication regimens
- Identify care gaps and provide actionable recommendations; treatment planning and medication prescribing are not required
Qualifications:
- Active and unrestricted Nurse Practitioner license in Washington
- Board Certification through AANP or ANCC
- Current BLS/CPR certification
- Minimum of 6 months experience as a Nurse Practitioner preferred
- MSN from an accredited nursing program
Compensation and Perks:
- Earn $80$90 per completed evaluation
- Mileage stipend and compensation for administrative time
