Deliver high-quality, compassionate care in a flexible, per diem setting by performing home health risk assessments for Medicare Advantage plan members throughout Hendry County. In this Nurse Practitioner role, youll have the opportunity to make a meaningful impact on patients well-being while enjoying the autonomy to create your own work schedule.

Schedule:

• Flexible shifts available Monday through Sunday
• Design your own schedule and select preferred days
• Commit to minimum weekly or bi-weekly hours as desired

Key Responsibilities:

• Perform comprehensive annual risk assessments for Medicare Advantage members in patients homes
• Gather and review medical history, current conditions, and medication details
• Educate patients about their health conditions and medication regimens
• Identify and document care gaps, offering practical recommendations to support continuity of care
• Focus solely on assessmentno treatment planning or medication prescribing involved

Qualifications:

• Active Nurse Practitioner license in Florida
• Current BLS/CPR certification
• Board certification via AANP or ANCC
• At least 6 months of recent experience as a Nurse Practitioner (preferred)
• Master of Science in Nursing (MSN) from an accredited institution

Receive competitive pay of $80$90 per completed evaluation, with mileage and administrative time also compensated.
