Job Description

This per diem opportunity empowers experienced Nurse Practitioners to conduct home health risk assessments for Medicare Advantage plan members across various locations in Hendry County. Schedule:

  • Flexible shifts available Monday through Sunday
  • Choose your own working hours
  • Set a preferred minimum weekly or bi-weekly hourly commitment

Responsibilities:

  • Perform comprehensive home health risk assessments during scheduled visits
  • Review and document prior and current medical histories, medication lists, and chronic conditions
  • Educate patients regarding their health conditions and prescribed medications
  • Recognize gaps in care and provide actionable recommendations to improve patient outcomes
  • Visits last approximately 45 minutes and may include both annual assessments and post-discharge follow-ups
  • Prescribing medications or managing treatment plans is not part of this role

Qualifications:

  • Active Nurse Practitioner license in Florida
  • Board Certification via AANP or ANCC
  • Current BLS certification (CPR)
  • Master of Science in Nursing from an accredited program
  • At least six months experience practicing as a Nurse Practitioner preferred
