Empower patients and advance your clinical skills as a Nurse Practitioner specializing in home health risk assessments across Brazos County. In this per diem role, youll serve Medicare Advantage plan members residing in communities such as College Station, Bryan, Navasota, and Wixon Valley.

Schedule:

• Flexible shifts available Monday through Sunday
• Customize your work schedule to suit your availability
• Minimum weekly or bi-weekly hourly commitment

Key Responsibilities:

• Conduct thorough annual home health risk assessments in members' homes; each evaluation typically lasts about 45 minutes
• Review and assess both prior and current medical history, chronic conditions, and medication use
• Educate patients on their health conditions and medication regimens to promote optimal wellness
• Detect gaps in care and offer guidance to help members and their healthcare teams close those gaps
• Focus on assessment and educationno need to prescribe medications or initiate treatment plans

Qualifications:

• Active Nurse Practitioner license in Texas
• Master of Science in Nursing from an accredited institution
• Board certification via AANP or ANCC
• Current CPR certification (BLS)
• At least 6 months of recent Nurse Practitioner experience preferred
