Experienced Nurse Practitioners with a background in home health are invited to deliver high-impact care through home health risk assessments. In this per diem opportunity, you will conduct visits for Medicare Advantage plan members throughout Yuma County, including Somerton, Yuma, and San Luis. These assessments are typically completed in approximately 45 minutes and can range from annual health checkups to post-discharge evaluations.

Key Responsibilities:

• Complete thorough home health risk assessments for Medicare Advantage members
• Review and document prior and current medical history, conditions, and medications
• Provide education on health status, chronic conditions, and treatments
• Identify gaps in care and make recommendations to improve patient outcomes
• Coordinate with care teams as neededno prescribing or treatment planning required

Qualifications:

• Active Nurse Practitioner license in Arizona
• Board Certified through AANP or ANCC
• Current CPR certification (BLS)
• MSN degree from an accredited nursing program
• A minimum of 6 months of recent NP experience preferred

Compensation for this role offers $80$90 per evaluation plus a mileage stipend and payment for administrative time.
