As a Nurse Practitioner specializing in home health, you will conduct comprehensive risk assessments in patients homes, ensuring quality healthcare access beyond the clinical setting.

Schedule:

• Flexible shifts, Monday through Sunday
• Design your own work schedule
• Weekly or bi-weekly minimum hourly commitments

Responsibilities:

• Conduct annual home health risk assessments for members of a Medicare Advantage plan
• Assess and document prior and current medical histories, conditions, and medications
• Offer patient education regarding medical conditions and medication management
• Identify gaps in care and provide recommendations to address them
• Adhere strictly to evaluation, education, and documentation; prescribing and treatment planning are not required
• Each evaluation averages 45 minutes, including administrative time and travel across various communities within St. Croix County (e.g., Hudson, River Falls, and New Richmond)

Qualifications:

• Active Nurse Practitioner license in Wisconsin
• Current Basic Life Support (CPR) certification
• Board Certified (AANP or ANCC)
• Master of Science in Nursing (MSN) from an accredited institution
• At least six months of current experience as an NP is preferred
