Take the lead in delivering comprehensive home health risk assessments as a Nurse Practitioner on a flexible, per diem basis. Provide crucial support to Medicare Advantage plan members across Chattanooga, East Ridge, Soddy-Daisy, and surrounding areas of Hamilton County, TN. This role allows you to create your own schedule, manage a variety of cases, and truly make a difference in your community.
Schedule:
- Flexible shifts available Monday through Sunday
- Design your own work schedule
- Choose your own minimum weekly or bi-weekly hourly commitment
Role Responsibilities:
- Perform annual and post-discharge home health risk assessments, dedicating approximately 45 minutes to each visit
- Review and document medical history, current conditions, and medication usage
- Deliver clear education to members regarding their health conditions and prescribed medications
- Identify care gaps and recommend strategies for improvement
- Focus exclusively on assessmentno treatment planning or prescribing required
Qualifications:
- Active Nurse Practitioner License in Tennessee
- Current CPR certification (BLS)
- Board Certification via AANP or ANCC
- Six months or more recent NP experience preferred
- Masters degree in Nursing (MSN) from an accredited institution
