This per diem nurse practitioner opportunity allows you to focus exclusively on comprehensive home health risk evaluations for Medicare Advantage plan members, supporting improved patient outcomes and care continuity.

Schedule:

• Flexible shifts available Monday through Sunday
• Design your own work schedule
• Minimal weekly or bi-weekly commitment requirements

Key Responsibilities:

• Conduct thorough annual or post-discharge home health risk assessments in patient residences
• Review medical histories, current conditions, and medication regimens
• Deliver educational guidance to patients on health conditions and proper medication use
• Identify care gaps and make recommendations for improved health management
• Note: This position does not require prescribing or developing treatment plans

Qualifications:

• Active Nurse Practitioner license in Tennessee
• Current CPR certification (BLS)
• Board Certification through AANP or ANCC
• Minimum of 6 months experience as a nurse practitioner preferred
• Master of Science in Nursing (MSN) from an accredited institution

Per evaluation, compensation ranges from $80 to $90, with a mileage stipend and administrative time included.
