Job Description

Advance your career as a Nurse Practitioner providing home health risk assessments for Medicare Advantage plan members in Jackson County. This per diem opportunity allows for flexibility and autonomy, as you work with individuals in their homes, performing crucial assessments that support optimal patient care. The workweek spans Monday through Sunday, and you have the freedom to design your own schedule based on your availability and preferred commitmentwhether weekly or bi-weekly.

Schedule:

  • Flexible shifts available any day, Monday through Sunday
  • Ability to create your own schedule
  • Minimum weekly or bi-weekly hourly commitment required

Key Responsibilities:

  • Perform home health risk assessments for Medicare Advantage plan members across Jackson County
  • Evaluate patients medical history, chronic conditions, and medication usage
  • Educate patients regarding their conditions and prescribed medications
  • Identify potential gaps in patient care, recommending appropriate follow-ups or interventions
  • Focus assessments on annual health visits or post-discharge needs (no ongoing treatment planning or prescribing)
  • Each assessment averages 45 minutes

Qualifications:

  • Current Nurse Practitioner license in Florida
  • BLS CPR certification
  • Board certification via AANP or ANCC
  • Minimum of six months recent NP experience preferred
  • Master of Science in Nursing (MSN) from an accredited institution

This role offers compensation of $80$90 per completed evaluation, with an added stipend for mileage and administrative time. The variety of locationsincluding Marianna, Graceville, and Sneadswithin Jackson County provides the chance to care for diverse individuals and communities.
