Key Responsibilities:

• Perform comprehensive annual home health risk assessments for Medicare Advantage members in various locations across Douglas County
• Assess clients prior and current medical history, medical conditions, and medication use
• Provide education tailored to each patient's medical conditions and medication regimen
• Identify gaps in care and offer clear recommendations for improvement
• Focus exclusively on assessment; there is no requirement to create treatment plans or prescribe medications

Qualifications:

• Active Nurse Practitioner License in Georgia
• CPR certification (BLS) required
• Board Certification via AANP or ANCC
• At least 6 months of recent Nurse Practitioner experience preferred
• MSN degree from an accredited school of nursing
