Advance your career as a Nurse Practitioner specializing in home health risk assessments, providing vital services to Medicare Advantage members throughout Kent County, DE. This per diem opportunity offers a high level of autonomy and flexibility, allowing you to make a meaningful impact while maintaining work-life balance.
Schedule:
- Flexible shifts available Monday through Sunday
- Ability to design and manage your own schedule
- Commitment can be set weekly or bi-weekly with a minimum hourly requirement
Key Responsibilities:
- Complete comprehensive annual health and risk assessments for Medicare Advantage members in their homes
- Evaluate medical histories, current conditions, and medication usage
- Educate patients on health conditions and proper medication use
- Spot and report potential gaps in care, providing actionable recommendations
- Assignments may include annual assessments or post-discharge visits with an average visit lasting about 45 minutes
- All assessments are based in Dover, Smyrna, Milford, and other areas across Kent County
- No need to develop treatment plans or prescribe medications
Qualifications:
- Active Nurse Practitioner license in Delaware
- Current BLS (CPR) certification
- Board certification through AANP or ANCC
- MSN degree from an accredited institution
- At least 6 months of recent Nurse Practitioner experience preferred
Compensation & Benefits: This role pays $80$90 per completed evaluation, and includes a mileage stipend along with compensation for administrative duties.
