Job Description

Advance your career as a Nurse Practitioner specializing in home health risk assessments, providing vital services to Medicare Advantage members throughout Kent County, DE. Schedule:

  • Flexible shifts available Monday through Sunday
  • Ability to design and manage your own schedule
  • Commitment can be set weekly or bi-weekly with a minimum hourly requirement

Key Responsibilities:

  • Complete comprehensive annual health and risk assessments for Medicare Advantage members in their homes
  • Evaluate medical histories, current conditions, and medication usage
  • Educate patients on health conditions and proper medication use
  • Spot and report potential gaps in care, providing actionable recommendations
  • Assignments may include annual assessments or post-discharge visits with an average visit lasting about 45 minutes
  • All assessments are based in Dover, Smyrna, Milford, and other areas across Kent County
  • No need to develop treatment plans or prescribe medications

Qualifications:

  • Active Nurse Practitioner license in Delaware
  • Current BLS (CPR) certification
  • Board certification through AANP or ANCC
  • MSN degree from an accredited institution
  • At least 6 months of recent Nurse Practitioner experience preferred

Compensation & Benefits: This role pays $80$90 per completed evaluation, and includes a mileage stipend along with compensation for administrative duties.
