Job Description
Provide essential care and make a difference in patients' lives as a Nurse Practitioner specializing in home health risk assessments. This per diem opportunity offers you the flexibility to design your own schedule and deliver personalized healthcare in Warren County, TNincluding McMinnville, Morrison, and Centertown. Youll perform evaluations for Medicare Advantage plan members, focusing on assessment and education, without the responsibility of developing treatment plans or prescribing medications.
Schedule:
- Flexible shifts offered seven days a week
- Build your own schedule to fit your lifestyle
- Minimum weekly or bi-weekly hourly commitment required
Key Responsibilities:
- Conduct in-home annual risk assessments for Medicare Advantage health plan members
- Review prior and current medical history, conditions, and medication use
- Educate patients on their health conditions and prescribed medications
- Identify care gaps and make recommendations to ensure optimal patient outcomes
- Travel within Warren County to various locations as assigned per shift
- Each assessment averages 45 minutes in duration
Qualifications:
- Active Nurse Practitioner License in Tennessee
- Current BLS (CPR) certification
- Board certified through AANP or ANCC
- MSN from an accredited nursing program
- Minimum of 6 months recent NP experience preferred
