Deliver exceptional patient care as a Nurse Practitioner in a flexible per diem home health role serving Medicare Advantage members in Warren County, TN. This independent position allows you to design your own schedule, completing risk assessments in patient homes throughout McMinnville, Morrison, Centertown, and nearby areas.
Schedule:
- Flexible shifts available Monday through Sunday
- Choose your own workdays and times
- Minimum weekly or bi-weekly hours based on your availability
Key Responsibilities:
- Perform comprehensive home health risk assessments with an average duration of 45 minutes per patient
- Review and document patients medical histories, current conditions, and medication lists
- Educate members on their health conditions and prescribed medications
- Identify care gaps and make recommendations for improvement without developing treatment plans or prescribing medications
- Complete visits that may range from annual wellness assessments to post-discharge follow-up, adapting to each members healthcare needs
Qualifications:
- Valid Nurse Practitioner (NP) license in Tennessee
- Board certification through AANP or ANCC
- Master of Science in Nursing (MSN) from an accredited institution
- Active CPR (BLS) certification
- At least six months of recent Nurse Practitioner experience preferred
