Deliver advanced patient care as a Nurse Practitioner specializing in home health, performing risk assessments for Medicare Advantage members in Harnett County, NC. This per diem opportunity allows you to support patient wellbeing through flexible scheduling, with shifts available any day of the week and the autonomy to set your own work hours. Assignments may include locations such as Dunn, Lillington, Anderson Creek, and Spout Springs, providing valuable in-home health evaluations that take about 45 minutes each.

Schedule:

• Flexible shifts between Monday and Sunday
• Create your own schedule
• Minimum weekly or bi-weekly hourly commitment

Key Responsibilities:

• Conduct comprehensive home health risk assessments for Medicare Advantage plan members
• Evaluate patients prior and current medical history, conditions, and medications
• Educate patients about their conditions, treatments, and medications
• Identify gaps in care and provide actionable recommendations to improve health outcomes
• Each assessment is focused on evaluation; you will not be required to create treatment plans or prescribe medications

Qualifications:

• Active Nurse Practitioner license in NC
• Board certification through AANP or ANCC
• MSN from an accredited nursing program
• Current CPR certification (BLS)
• At least 6 months of recent Nurse Practitioner experience preferred
