Job Description

Take on a dynamic per diem opportunity as a Nurse Practitioner specializing in home health risk assessments. This position involves visiting Medicare Advantage plan members across various communities within Harnett County, providing valuable wellness assessments and care insights right in patients' homes. Flexible scheduling allows you to create your own hours, with minimum weekly or bi-weekly hour commitments to fit your lifestyle.

Key Responsibilities:

  • Perform comprehensive home health risk assessments (approximately 45 minutes per visit) for Medicare Advantage health plan members
  • Review and evaluate prior and current medical history, chronic conditions, and medication records
  • Deliver patient education on health conditions and prescribed medications
  • Detect gaps in patient care and recommend steps for improvement
  • Visits may include annual health assessments or post-discharge follow-ups
  • No requirement for treatment plan creation or medication prescribing

Qualifications:

  • Active Nurse Practitioner License in North Carolina
  • MSN degree from an accredited nursing school
  • Board Certification (AANP or ANCC)
  • Current CPR (BLS) certification
  • Minimum of 6 months of recent NP experience preferred

Enjoy compensation of $80-$90 per completed evaluation, along with a mileage stipend and payment for administrative time. You will serve members in areas including Dunn, Lillington, Anderson Creek, Spout Springs, and the surrounding communities, with assignments scheduled flexibly based on regional needs.
