Advance your career as a Nurse Practitioner specializing in home health by conducting risk assessments for Medicare Advantage plan members throughout Ouachita County, AR. In this per diem opportunity, you will visit patients at their homes, supporting quality care with flexible scheduling and valuable clinical impact.

Schedule:

• Flexible shifts available Monday through Sunday
• Choose your own work days and hours
• Commitment of minimum hours per week or every two weeks as mutually agreed

Key Responsibilities:

• Perform home health risk assessments, each approximately 45 minutes in duration
• Review and evaluate patients medical histories, existing conditions, and medication usage
• Educate members on their diagnosed health conditions and medications
• Identify potential gaps in care and provide actionable recommendations
• Deliver annual health assessments and post-discharge follow-up visits
• All visits are assessment onlyno treatment planning or medication prescribing required

Desired Qualifications:

• Active Nurse Practitioner license in Arkansas
• Board certification via AANP or ANCC
• Current BLS (CPR) certification
• MSN from an accredited nursing school
• Minimum six months of recent Nurse Practitioner experience preferred

Compensation for this role is a competitive per evaluation rate ranging from $80$90, in addition to a mileage stipend and administrative time, providing robust earning potential while supporting patient care in the community.
