Schedule:
- Flexible shifts available Monday through Sunday
- Self-scheduling: set your own calendar
- Minimum hourly commitment on either a weekly or bi-weekly basis
Key Responsibilities:
- Perform annual home health risk assessments for members enrolled in a Medicare Advantage health plan
- Assess medical history, existing conditions, and current/past medication usage
- Provide thorough patient education related to health conditions and prescribed medications
- Identify gaps in care, offer recommendations to close those gaps, and document findings
- Each evaluation typically takes about 45 minutes; visits may include annual assessments or post-discharge follow-ups
- No development of treatment plans or prescription duties involved
Qualifications:
- Current Nurse Practitioner license in the state of Arkansas
- Active Basic Life Support (BLS/CPR) certification
- Board Certification as a Nurse Practitioner (AANP or ANCC)
- MSN degree earned from an accredited nursing program
- Minimum of 6 months of recent experience as an NP preferred
