Deliver valuable care directly to patients by performing home health risk assessments for Medicare Advantage plan members across Cayuga County. This per diem opportunity empowers Nurse Practitioners to utilize their expertise in real-world settingsincluding communities such as Auburn, Cayuga, Union Springs, and Moraviamaking a direct impact on patient wellbeing without the demands of ongoing treatment management or prescribing.

Schedule:

• Flexible shifts available any day, Monday through Sunday
• Self-scheduling: tailor your hours and commitment weekly or bi-weekly

Key Responsibilities:

• Conduct comprehensive annual and post-discharge home health risk assessments
• Review prior and current medical histories, health conditions, and medication regimens
• Educate members about their medical conditions and medications to promote understanding and adherence
• Identify care gaps, recommending solutions to address unmet health needs
• Each home visit typically lasts about 45 minutes; travel to patient residences is required
• No prescribing or development of long-term treatment plans

Preferred Qualifications:

• Current Nurse Practitioner license in New York State
• Board certified through AANP or ANCC
• Active CPR (BLS) certification
• MSN from an accredited nursing program
• At least 6 months of recent experience practicing as a Nurse Practitioner highly preferred
