An exciting per diem opportunity is available for a Nurse Practitioner skilled in home health assessments. This role is based in Deschutes County and surrounding communities including Bend, Redmond, Deschutes River Woods, Three Rivers, La Pine, and Sisters. Youll have the flexibility to design your own schedule, providing crucial care in a rewarding, patient-focused environment.

Schedule:

• Flexible shifts available Monday through Sunday
• Set your own working hours each week
• Minimum weekly or bi-weekly hourly commitment

Responsibilities:

• Perform home health risk assessments on Medicare Advantage plan members, typically lasting about 45 minutes per assessment
• Review and assess prior and current medical history, medical conditions, and medication usage
• Offer valuable education regarding health conditions and medications to patients
• Detect any care gaps and deliver recommendations to enhance patient outcomes
• Visits may include annual health reviews or post-discharge assessments
• No requirement to create treatment plans or prescribe medications

Qualifications:

• Active Nurse Practitioner license in Oregon
• Current Basic Life Support (BLS) certification
• Board certification through AANP or ANCC
• Minimum of six months of recent NP experience preferred
• MSN degree from an accredited nursing program
