Deliver exceptional patient-centered care as a Nurse Practitioner specializing in home health risk assessments. This per diem opportunity caters to Medicare Advantage plan members throughout Nueces County, TXincluding Corpus Christi, Agua Dulce, Driscoll, Port Aransas, and surrounding areas. Schedule:

• Flexible shifts available seven days a week
• Create your own schedule to match your availability
• Minimum weekly or bi-weekly hourly commitment as preferred

Key Responsibilities:

• Conduct annual or post-discharge home health risk assessments for Medicare Advantage members in their residences
• Review and evaluate patients medical history, medications, and ongoing health conditions
• Educate patients on their conditions, medication management, and the importance of preventive care
• Identify gaps in care and provide actionable recommendations to optimize patient outcomes
• Focus exclusively on assessmentno medication prescribing or treatment planning required

Qualifications:

• Active Nurse Practitioner license in Texas
• Board Certification through AANP or ANCC
• Current CPR certification (BLS required)
• MSN degree from an accredited school of nursing
• At least 6 months of current NP experience preferred

You will receive $80$90 per evaluation, in addition to a mileage stipend and compensation for administrative time, reinforcing the value placed on your expertise and commitment.
