This per diem opportunity offers nurse practitioners the flexibility to make an impact through home health risk assessments across Grant County, NM, including Bayard-Santa Rita, Santa Clara, Tyrone, Hurley, and the wider Silver City area. As a trusted healthcare provider, you will visit Medicare Advantage plan members in their homes, ensuring comprehensive assessments and personalized support.

Schedule:

• Flexible shifts available Monday through Sunday
• Design your own work schedule
• Choose your preferred minimum weekly or bi-weekly hours

Key Responsibilities:

• Perform annual and post-discharge home health risk assessments for Medicare Advantage members
• Review and evaluate prior/current medical history, chronic conditions, and medication adherence
• Offer patient education regarding disease management and prescribed medications
• Identify care gaps and recommend follow-up actions to enhance health outcomes
• Focus exclusively on assessment and educationno prescribing or treatment planning required

Qualifications:

• Active Nurse Practitioner license in New Mexico
• Current Basic Life Support (BLS) CPR certification
• Board certification via AANP or ANCC
• Master of Science in Nursing (MSN) from an accredited program
• At least 6 months of recent nurse practitioner experience preferred
