This per diem opportunity offers flexibility and autonomy for dedicated Nurse Practitioners committed to making a difference in home health. You will perform risk assessments and health evaluations for Medicare Advantage plan members throughout Garfield County, including Glenwood Springs, Rifle, Carbondale, and surrounding areas.
Schedule:
- Flexible shifts available Monday through Sunday
- Create your own schedule to fit your lifestyle
- Minimum weekly or bi-weekly hourly commitment
Key Responsibilities:
- Conduct comprehensive annual home health risk assessments for Medicare Advantage plan members
- Review and evaluate patients prior and current medical histories, chronic conditions, and medications
- Deliver thorough patient education regarding health conditions and medication management
- Identify gaps in care and provide thoughtful recommendations for follow-up
- Focus solely on assessmentsno prescribing of medication or creation of treatment plans required
Qualifications:
- Active Nurse Practitioner license in Colorado
- Valid CPR certification (BLS)
- Board certification through AANP or ANCC
- Minimum of six months current experience as a Nurse Practitioner preferred
- MSN degree from an accredited nursing program
