Deliver advanced patient care and make a positive impact in your community as a Nurse Practitioner providing home health risk assessments. This per diem opportunity allows for a highly flexible schedule, making it ideal for clinicians seeking autonomy and work-life balance. As a crucial part of healthcare delivery, you will serve patients residing throughout Randolph County, including Asheboro, Archdale, and Trinity.
Schedule:
- Flexible shifts available Monday through Sunday
- Ability to create your own schedule
- Minimum weekly or bi-weekly hourly commitment
Essential Responsibilities:
- Complete thorough annual home health risk assessments for Medicare Advantage plan members
- Review and evaluate prior and current medical history, assess medical conditions, and review medication usage
- Educate patients about their conditions, overall health, and prescribed medications
- Detect care gaps and make recommendations based on assessment findings
- Focus exclusively on assessmentno development of treatment plans or medication prescribing required
Qualifications:
- Active Nurse Practitioner license in North Carolina
- Current BLS CPR certification
- Board certified through AANP or ANCC
- MSN from an accredited nursing program
- At least 6 months of recent Nurse Practitioner experience preferred
