Deliver high-quality patient care in a dynamic role focused on home health risk assessments for Medicare Advantage plan members across Hamilton County, TN. As an experienced Nurse Practitioner, you will visit patients in their homes to perform comprehensive risk assessmentseach lasting around 45 minutestailored to annual health evaluations or post-discharge needs. Assignments may cover Chattanooga, East Ridge, Soddy-Daisy, and other nearby communities to ensure comprehensive patient outreach within the county.

Schedule:

• Flexible shifts available Monday through Sunday
• Set your own hours and build a personalized work schedule
• Minimum hourly commitments can be weekly or bi-weekly

Key Responsibilities:

• Complete annual home health risk assessments for assigned Medicare Advantage plan members
• Review, evaluate, and document current and prior medical history, health conditions, and medication use
• Educate patients on diagnosis, treatment options, and medication regimen to support wellness
• Detect gaps in care and offer recommendations that contribute to improved health outcomes
• Note: This position does not require writing treatment plans or prescribing medications

Qualifications:

• Active Nurse Practitioner license in the state of TN
• Valid BLS certification (CPR)
• Board Certified by AANP or ANCC
• MSN degree from an accredited nursing program
• Minimum 6 months experience preferred as a Nurse Practitioner in a clinical setting
