Deliver high-quality care in a rewarding, autonomous environment as a Nurse Practitioner specializing in home health risk assessments. This per diem opportunity empowers you to impact patient outcomes for Medicare Advantage members while enjoying maximum schedule flexibility.
Schedule:
- Flexible shifts available between Monday and Sunday

- Minimum weekly or bi-weekly hourly commitments
Key Responsibilities:
- Complete annual home health risk assessments for Medicare Advantage plan members in Hamilton County, including Chattanooga, East Ridge, and Soddy-Daisy
- Review comprehensive medical histories, current health conditions, and medication usage
- Educate patients about their medical conditions and medications
- Identify gaps in care, making actionable recommendations to enhance care quality
- No treatment plans or medication prescribing required during visits
Qualifications:
- Active Nurse Practitioner (NP) license in Tennessee
- Valid CPR certification (BLS)
- Board Certification via AANP or ANCC
- Minimum of six months recent experience as an NP preferred
- Master of Science in Nursing (MSN) from an accredited nursing program
Enjoy competitive compensation of $80-$90 per evaluation, supplemented by a mileage stipend and remuneration for administrative tasks. Conduct visits that average approximately 45 minutes each, serving a diverse patient population throughout Hamilton County.
