This per diem opportunity is focused within Shelby County, with flexibility to serve Sidney, Anna, Fort Loramie, Russia, and neighboring communities. Schedule:
- Flexible shifts available Monday through Sunday
- Create your own work schedule
- Minimum weekly or bi-weekly hourly commitment required
Key Responsibilities:
- Conduct comprehensive home health risk assessments, typically lasting about 45 minutes per visit
- Assess prior and current medical histories, chronic conditions, and medication use
- Deliver patient and caregiver education regarding health conditions and medication management
- Identify gaps in patient care and recommend appropriate next steps to promote wellness
- No need to develop treatment plans or prescribe medications
Qualifications:
- Active Ohio Nurse Practitioner license
- Board Certification (AANP or ANCC)
- Master of Science in Nursing (MSN) from an accredited program
- Current BLS/CPR certification
- At least six months of recent NP experience preferred
