Job Description
Make a positive impact in community healthcare by performing specialized home health risk assessments. This per diem opportunity is perfect for motivated Nurse Practitioners seeking flexibility and autonomy while working with Medicare Advantage plan members in Sidney, OH and surrounding areas within Shelby County, including Anna, Fort Loramie, and Russia.
Schedule:
- Flexible shifts available between Monday and Sunday
- Self-scheduling: Create your own work schedule
- Minimum weekly or bi-weekly hourly commitment, tailored to your availability
Key Responsibilities:
- Complete annual home health risk assessments for Medicare Advantage members in their residences
- Review and evaluate patients medical histories, current medical conditions, and medication regimens
- Educate patients regarding health conditions and medication management
- Identify care gaps, making practical recommendations for follow-up and resolution
- There is no requirement for developing treatment plans or prescribing medications
Qualifications:
- Unrestricted Nurse Practitioner license in Ohio
- Active CPR certification (BLS)
- Board Certification through AANP or ANCC
- Minimum of 6 months recent Nurse Practitioner experience preferred
- MSN degree from an accredited nursing program
Youll receive compensation of $80$90 per evaluation, along with mileage reimbursement and payment for administrative time. This is a per-visit, field-based role covering different locations in Shelby County as scheduled.
