Job Description
As a Nurse Practitioner specializing in home health, youll play a critical role conducting health risk assessments in Miami County, Ohioincluding Troy, Piqua, Tipp City, Union, and surrounding communities. This per diem opportunity is designed for those who value both autonomy and impact, serving Medicare Advantage members in the comfort of their homes.
Schedule:
- Flexible shifts, Monday through Sunday
- Build your own schedule
- Commit to minimum weekly or bi-weekly hours based on your availability
Key Responsibilities:
- Perform annual home health risk assessments for Medicare Advantage plan members
- Assess medical history, current conditions, and review medication regimens
- Educate patients on diagnoses, preventive care, and medication usage
- Identify gaps in care, offering recommendations to enhance outcomes
- No requirements for prescribing or generating treatment plans
- Most home visits typically last 45 minutes
- Serve a diverse region within Miami County, with each shift assigned to varying localities
Qualifications:
- Active Nurse Practitioner license in the state of Ohio
- Board certified through either AANP or ANCC
- Current BLS/CPR certification
- MSN degree from an accredited program
- At least 6 months of recent NP experience in any care setting (preferred)
This position offers compensation of $80$90 per completed evaluation, along with a mileage stipend and administrative time, supporting you for both time and travel.
