Job Description
Deliver high-quality patient assessments in a flexible, per diem home health setting. As a Nurse Practitioner, you will perform comprehensive risk assessments for Medicare Advantage plan members throughout Miami County, Ohioincluding areas like Troy, Piqua, Tipp City, and Union. Each evaluation takes approximately 45 minutes and may be an annual health assessment or a post-discharge visit. Enjoy autonomy, diverse daily locations, and the opportunity to impact members' health from the comfort of their homes.
Schedule:
- Flexible shifts available Monday through Sunday
- Option to create your own schedule
- Minimum weekly or bi-weekly hourly commitment
Key Responsibilities:
- Conduct annual risk assessments in members homes
- Review and evaluate patients past and current medical history, conditions, and medication use
- Educate members on their health conditions and prescribed medications
- Identify gaps in care and recommend appropriate next steps
- No development of treatment plans or prescribing of medications required
Qualifications:
- Active Nurse Practitioner license in Ohio
- Board Certification from AANP or ANCC
- MSN degree from an accredited nursing school
- Current CPR (BLS) certification
- At least 6 months of recent NP experience preferred
