Deliver vital healthcare services in a unique, patient-centered setting as a Nurse Practitioner specializing in home health risk assessments. In this per diem position, flexibility is at the forefront, allowing you to design your own schedule and make a meaningful impact on Medicare Advantage plan members residing throughout Yamhill County, OR, including Newberg, Dundee, and McMinnville.

Schedule:

• Flexible shifts available seven days a week
• Create your own schedule based on your availability
• Choose between minimum weekly or bi-weekly hourly commitments

Key Responsibilities:

• Conduct comprehensive annual or post-discharge home health risk assessments for Medicare Advantage members in their residences
• Assess patients current and past medical history, existing conditions, and medication lists
• Educate patients on their medical conditions and prescribed medications
• Identify gaps in care and provide actionable recommendations for care enhancement
• No requirement to prescribe medication or develop treatment plans

Qualifications:

• Active Nurse Practitioner license in Oregon
• Current BLS CPR certification
• Board Certification through AANP or ANCC
• Minimum six months of recent Nurse Practitioner experience preferred
• MSN degree from an accredited nursing school

Compensation and Benefits: Compensation for this per diem position is $80$90 per evaluation, plus a mileage stipend and compensation for administrative time.
