Step into a dynamic, patient-centered role supporting diverse communities within Yamhill County through comprehensive home health risk assessments. As a Nurse Practitioner specializing in home health, youll engage directly with Medicare Advantage members to assess medical needs and foster better health outcomeswithout the demands of ongoing treatment or prescribing responsibilities.

Schedule:

• Flexible shifts available Monday through Sunday
• Set your own schedule
• Minimum weekly or bi-weekly hourly commitments

Primary Responsibilities:

• Perform annual home health risk assessments for Medicare Advantage members in various locations within Yamhill County, such as McMinnville, Newberg, and Dundee.
• Review and evaluate patients prior and current medical histories, existing conditions, and medication regimens.
• Offer clear, informative education about each members conditions and medications.
• Identify and document any care gaps, providing recommendations for closing those gaps.
• Complete each risk assessment in approximately 45 minutes with a focus on efficiency and compassionate care.
• No responsibilities for creating treatment plans or prescribing medications.

Qualifications:

• Active Nurse Practitioner License in Oregon
• Board Certification via AANP or ANCC
• Current CPR certification (BLS)
• MSN degree from an accredited nursing program
• At least 6 months of recent Nurse Practitioner experience preferred, though not required
