Advance your clinical skills and support patient wellness through individualized care in a flexible per diem opportunity as a Nurse Practitioner specializing in home health risk assessments. This role involves traveling to Medicare Advantage plan members residences throughout Bonneville County, with visits that may include annual health assessments or post-discharge check-ins. Each appointment lasts approximately 45 minutes, providing the chance to make a meaningful impact on patients health from Idaho Falls to Ammon and Iona.

Schedule:

• Flexible shifts offered between Monday and Sunday
• Self-scheduling to accommodate your availability
• Minimum weekly or bi-weekly hour requirements

Key Responsibilities:

• Perform comprehensive annual risk assessments for Medicare Advantage enrollees in their homes
• Review and evaluate prior and current medical history, existing health conditions, and medication regimens
• Educate patients on their health conditions and prescribed treatments
• Assess for gaps in care and recommend appropriate next steps
• No ongoing treatment plan development or medication prescribing is required

Qualifications:

• Active Nurse Practitioner license in Idaho
• Current BLS/CPR certification
• Board certification through AANP or ANCC
• At least 6 months of recent NP experience (preferred)
• MSN degree from an accredited nursing program
