Advance your career as a Nurse Practitioner specializing in home health risk assessments with flexible, per diem opportunities. You will travel to various locations within Coffee Countyincluding Tullahoma, Manchester, and Beechgroveto visit Medicare Advantage plan members in their homes. This role emphasizes autonomy and meaningful patient connections without the need to diagnose or prescribe.
Schedule:
- Flexible shifts available Monday through Sunday
- Set your own schedule
- Commit to a minimum number of hours weekly or bi-weekly
Key Responsibilities:
- Conduct comprehensive annual and post-discharge home health risk assessments for Medicare Advantage members
- Evaluate patients prior and current medical history, review their medical conditions, and assess medication histories
- Educate patients on their health conditions and medications to promote self-management
- Identify care gaps and make recommendations to help close those gaps
- No requirement for treatment plan development or medication prescribing
Qualifications:
- Current Nurse Practitioner license in Tennessee
- Board Certification from AANP or ANCC
- Active CPR/BLS certification
- Master of Science in Nursing (MSN) from an accredited institution
- Minimum of 6 months experience as a Nurse Practitioner preferred
