Job Description

Deliver impactful care in the home setting as a Nurse Practitioner specializing in home health risk assessments. In this per diem role, youll perform clinical evaluations for Medicare Advantage plan members across Coffee County, TNincluding Tullahoma, Manchester, and Beechgrove. Each patient visit averages about 45 minutes and may encompass an annual health assessment or a post-discharge evaluation.

Schedule:

  • Flexible schedulingchoose your shifts between Monday and Sunday
  • Set your own weekly or bi-weekly work hours
  • No minimum shift length; customize your commitment

Key Responsibilities:

  • Conduct comprehensive annual risk assessments in patients homes
  • Evaluate current and past medical history, conditions, and medication use
  • Deliver patient education on diagnoses, preventive health, and medication regimens
  • Identify any gaps in care, advising on next steps for improved health outcomes
  • Note: Prescribing medication and creating treatment plans are not required for this position

Qualifications:

  • Active Nurse Practitioner license in Tennessee
  • Current CPR certification (BLS)
  • Board certification via AANP or ANCC
  • At least six months' experience as a Nurse Practitioner preferred
  • MSN from an accredited nursing school

Enjoy compensation of $80$90 per evaluation, with a mileage stipend and additional pay for administrative time.
