Job Description

Deliver exceptional care directly to patients homes by performing comprehensive home health risk assessments for Medicare Advantage plan members in Coffee County, TN. This per diem opportunity gives Nurse Practitioners the flexibility to design their own schedules with shifts available throughout the week, including weekends. The role involves traveling to various locations within Coffee County, such as Tullahoma, Manchester, and Beechgrove, conducting thorough, patient-focused evaluations that average approximately 45 minutes each.

Schedule:

  • Flexible shifts available Monday through Sunday
  • Self-schedulingset your own hours
  • Minimum commitment required on a weekly or bi-weekly basis

Key Responsibilities:

  • Perform in-home annual health risk assessments for Medicare Advantage members
  • Review and document prior/current medical histories, existing conditions, and medication usage
  • Educate patients on their individual health conditions and prescribed medications
  • Identify potential care gaps and recommend follow-up actions
  • Deliver assessments only; no requirement for developing treatment plans or prescribing medications

Qualifications:

  • Active Nurse Practitioner license in Tennessee
  • Board certification (AANP or ANCC)
  • Current BLS/CPR certification
  • Masters degree in Nursing (MSN) from an accredited institution
  • Minimum of 6 months of recent NP experience preferred
