Take your Nurse Practitioner expertise into the community with a per diem opportunity focused on delivering home health risk assessments for Medicare Advantage plan members throughout Shelby County. Using your clinical skills, you'll have the flexibility to set your own schedule, working in areas such as Shelbyville, Fairland, and Morristown as needs arise. This role is ideal for nurses passionate about proactive healthcare and patient education, with each evaluation averaging 45 minutes and offering meaningful patient interaction.

Schedule:

• Flexible shifts available Monday through Sunday
• Self-scheduled appointments
• Minimum weekly or bi-weekly hourly commitment

Responsibilities:

• Conduct comprehensive home health risk assessments, including annual and post-discharge visits
• Evaluate patients medical history, current conditions, and medication regimen
• Educate patients on disease processes, preventative care, and prescribed medications
• Identify potential care gaps and advise on actionable next steps
• No requirement to initiate treatment plans or prescribe medications

Qualifications:

• Active Nurse Practitioner license in Indiana
• Board Certification via AANP or ANCC
• Current BLS/CPR certification
• MSN from an accredited nursing program
• At least 6 months of recent NP experience preferred
