Job Description

Make a meaningful impact as a Nurse Practitioner by delivering comprehensive home health risk assessments for Medicare Advantage plan members throughout Hancock County. This per diem opportunity offers full flexibilitydesign your own schedule with shifts available any day of the week. Youll visit a range of locations within Hancock County, such as Greenfield, McCordsville, and Fortville, to complete assessments that help bridge gaps in care and improve patient outcomes.

Schedule:

  • Flexible shifts offered Monday through Sunday
  • Craft your own schedule; ideal for work-life balance
  • Minimum commitment per week or bi-weekly

Key Responsibilities:

  • Perform annual risk assessments for Medicare Advantage members in their homes
  • Review and evaluate prior and current medical history, conditions, and medication usage
  • Educate patients on their health conditions and medications
  • Identify and report any gaps in care, offering recommendations to support better health outcomes
  • No need to create treatment plans or prescribe medication

Qualifications:

  • Active Nurse Practitioner (NP) License in Indiana
  • MSN from an accredited school of nursing
  • Current CPR certification (BLS)
  • Board certified through AANP or ANCC
  • At least 6 months of recent NP experience preferred
