Job Description

Deliver essential healthcare in the community by performing comprehensive home health risk assessments for Medicare Advantage plan members across Rutherford County, including Forest City, Spindale, and Rutherfordton. This per diem opportunity offers flexible scheduling with shifts available any day of the week, empowering you to tailor your work commitments while making a tangible difference.

Schedule:

  • Flexible shifts between Monday and Sunday
  • Create your own schedule
  • Minimum weekly or bi-weekly hourly commitment

Key Responsibilities:

  • Complete annual home health risk assessments during in-home visits, typically lasting approximately 45 minutes
  • Review and evaluate patients' medical histories, chronic and acute conditions, and current medications
  • Educate patients on their health conditions and medication regimens
  • Identify care gaps and recommend appropriate follow-up or intervention
  • No need to create treatment plans or prescribe medications

Qualifications:

  • Active Nurse Practitioner license in North Carolina
  • Current CPR (BLS) certification
  • Board certification via AANP or ANCC
  • Minimum of 6 months recent Nurse Practitioner experience preferred
  • Master of Science in Nursing (MSN) degree from an accredited nursing school
