Job Description

We're seeking an experienced Nurse Practitioner to perform home health risk assessments on a per diem basis throughout Seminole County, OK. This essential role involves visiting Medicare Advantage plan members in their residences, including areas such as Wewoka, Bowlegs, and Konawa. Each assessment typically takes about 45 minutes and may include annual health reviews or post-discharge visits. You will have the opportunity to directly influence patient outcomes by identifying unmet health needs and promoting effective care managementwithout the responsibilities of prescribing medications or managing ongoing treatment plans.

Key Responsibilities:

  • Conduct thorough annual or post-discharge home health risk assessments for Medicare Advantage members
  • Evaluate patients past and present medical histories, chronic conditions, and current medication usage
  • Educate patients on their health conditions and prescribed medications to encourage adherence and healthy behaviors
  • Identify gaps in care and make clinical recommendations to improve health outcomes
  • Ensure accurate and complete documentation in a timely manner

What Youll Need:

  • Active Nurse Practitioner license in Oklahoma
  • Current CPR (BLS) certification
  • Board certification through AANP or ANCC
  • MSN degree from an accredited nursing program
  • Minimum of 6 months recent NP experience preferred

You will be competitively compensated at $80-$90 per home health evaluation completed, and you'll also receive a mileage stipend along with administrative time coverage.
