Job Description

Make a meaningful impact as a Nurse Practitioner by supporting Medicare Advantage members through home health risk assessments in Seminole County, OK and the surrounding areas, including Wewoka, Bowlegs, and Konawa. This per diem role offers you autonomy and flexibility, with the opportunity to create your own schedule and select shifts throughout the week, including weekends.

Schedule:

  • Flexible shifts between Monday and Sunday
  • Self-scheduling to meet your availability
  • Minimum weekly or bi-weekly hourly commitment

Key Responsibilities:

  • Conduct comprehensive home health risk assessments for Medicare Advantage members
  • Review and evaluate patients prior and current medical history, ongoing medical conditions, and medication history
  • Offer education regarding medical conditions and medications to patients and their families
  • Identify care gaps and provide customized recommendations to facilitate improved patient outcomes
  • Note: Prescribing medications and developing treatment plans are not required

Qualifications:

  • Active Nurse Practitioner license in Oklahoma
  • Master of Science in Nursing (MSN) from an accredited program
  • Board Certified as a Nurse Practitioner (AANP or ANCC)
  • Valid CPR certification (BLS)
  • Minimum of 6 months of recent nurse practitioner experience is preferred
